Consumer Release for Power/Balance, Inc

By submitting my story online to Power/Balance, Inc, I hereby assign all rights to the words written herein my online submission. I hereby authorize Power/Balance, Inc, the use, reproduction, publishing, broadcast, and/or distribution of my image and remarks, without limitation, in perpetuity for use on all and any printed material, website, CD/DVD-ROM, or other new media developed in the future.

I hereby release, discharge, and agree to hold harmless Power/Balance, Inc, and their representatives, assign employees, or any person & persons acting under their permission or authority in whole or in part, from any part of all claims, including without limitation claims of privacy, publicity or defamation in the use or promotion of the likeness of recordings.

This agreement in entered into and governed by the laws of the state of Nevada and contains the entire agreement of the parties. Any disputes arising out of this agreement shall be adjudicated in the appropriate courts in Nevada.

I have read the foregoing release, authorization, and agreement before submitting my story and warrant that I fully understand the contents thereof.
